Community Support Services

 

Program

Community Support Services (CSS) provides community-based services for persons whose needs have not been met through traditional office-based treatment.

The CSS team approach assumes responsibility for directly providing acute, active, and ongoing community-based psychiatric treatment, assertive outreach, rehabilitation, and support.

Services are comprehensive and highly individualized, and are modified as needed through an ongoing assessment and treatment planning process. Services vary in intensity based on the persons served.

Persons Served
The CSS program provides psychosocial and intensive case management services directed primarily to adults (18 years and older) with severe and persistent mental illness who have a high use history of State Operated Psychiatric Facilities (SOF). They characteristically have frequent and repeated admissions with their stay at the SOF usually being for a prolonged period of time.

CSS clients often have co-occurring issues such as substance abuse, homelessness, legal issues, and/or lack of community and family support.

Treatment Team
A multidisciplinary treatment team consisting of a case-manager, a nurse, a psychiatrist, a substance abuse consultant, and a program coordinator provides CSS services.

Multiple members of the team are familiar with each person served to assure the timely and continuous provision of services.

The majority of services are provided directly by CSS team members with minimal referral to outside providers for the purpose of continuity of care. The team members are available on a 24 hours per day, 7 days per week, basis.

Services
CSS clients require a well coordinated and integrated package of services provided over an extended period of time for them to live successfully in the community. These services include, but are not limited to, the following:

* Budgeting/Representative Payee Services
* Support and Advocacy Services
* Crisis Intervention Services
